    In short

    What transition support includes

NICU-to-home support turns the hospital discharge plan into an assessed home arrangement for the baby and family. It can include skilled nursing, feeding support, equipment and appointment coordination, with the NICU team’s instructions and escalation route carried into the care plan.

    Who this is for

    • Babies preparing to leave NICU
    • Parents arranging skilled support
    • Families coordinating hospital follow-up
    • Newborns with documented care needs

    What is included

    1. 01

      Assessment and scope

An assessor reviews the person’s needs, home, routines and risks, then confirms the care plan, professionals and hours required.

    2. 02

      Records and escalation

Care is delivered at home only when the assessment, professional scope and home environment make it safe and appropriate.

    Care, by way of example

    • Hospital discharge transition

      from₦50,000

    One-off, before care starts

    The home assessment

    ₦35,000

A care coordinator visits once, before any care begins, to understand the needs and the home and agree the care plan with you. It is separate from the price of the care itself.

    Step by step

    How this works

    1. 1Postnatal care and Omugwo

The assessment separates clinical mother-and-baby care from broader Omugwo, night, meal and household support.

    2. 2The home assessment

An assessor reviews the person’s needs, home, routines and risks, then confirms the care plan, professionals and hours required.

    3. 3What can be delivered at home

Care is delivered at home only when the assessment, professional scope and home environment make it safe and appropriate.

    4. 4Escalation and the emergency boundary

Staff follow a documented escalation process, but emergencies must go directly to the emergency services.
