    In short

    The short answer

    You may need a nurse for wound care, prescribed medicines, observations or other clinical tasks; a caregiver for washing, dressing, meals, mobility and daily routines; and a physiotherapist where rehabilitation is part of the discharge plan. Some people need a coordinated combination rather than one professional.

    The home assessment

    A care coordinator visits once, before any care begins, to understand the needs and the home and agree the care plan with you. It is separate from the price of the care itself.

      An assessor reviews the person’s needs, home, routines and risks, then confirms the care plan, professionals and hours required.

    2. 2What can be delivered at home

      Care is delivered at home only when the assessment, professional scope and home environment make it safe and appropriate.

    3. 3What is recorded during care

      Managed care is recorded against the care plan and reviewed when needs, risks or circumstances change.

    4. 4Shifts, handover and supervision

      A rostered team covers the agreed hours, records each shift and gives a structured handover to the next professional.
