    In short

    The short answer

The equipment depends on the person’s discharge instructions, mobility, transfers, personal care, wound or treatment needs and the layout of the home. Common categories to assess are mobility and transfer aids, bathroom safety, pressure care, treatment supplies and monitoring equipment; do not buy equipment before the clinical and home requirements are confirmed.

    What to check

    • Ask the hospital team for written discharge and equipment instructions.
    • Check access, stairs, bathroom use, bed position and safe transfers at home.
    • Confirm who will supply, install, teach and maintain each item.

    Step by step

    How this works

    1. 1The home assessment

An assessor reviews the person’s needs, home, routines and risks, then confirms the care plan, professionals and hours required.

    2. 2What can be delivered at home

Care is delivered at home only when the assessment, professional scope and home environment make it safe and appropriate.

    3. 3How pricing works

The assessment is fixed-price; defined services use published prices, live-in care has a from price, and other ongoing and package care is quoted after assessment.

    4. 4What is recorded during care

Managed care is recorded against the care plan and reviewed when needs, risks or circumstances change.
