    In short

    The short answer

Prepare the home from the written discharge plan: make access and transfers safe, position the bed and essential equipment, obtain prescribed medicines and supplies, and confirm who will provide each visit or shift. Keep emergency and escalation contacts available before the person arrives.

    What to check

    • Remove access hazards and confirm safe bathroom and sleeping arrangements.
    • Arrange equipment, medicines, dressings and other prescribed supplies.
    • Confirm the care rota, follow-up appointments and escalation contacts.

    What it costs

    Indicative fees

A guide to what care typically costs. Your fee is confirmed in the care plan agreed after the assessment.

    Care, by way of example

      from₦50,000

    One-off, before care starts

    The home assessment

    ₦35,000

A care coordinator visits once, before any care begins, to understand the needs and the home and agree the care plan with you. It is separate from the price of the care itself.

    Step by step

    How this works

    1. 1The home assessment

An assessor reviews the person’s needs, home, routines and risks, then confirms the care plan, professionals and hours required.

    2. 2What can be delivered at home

Care is delivered at home only when the assessment, professional scope and home environment make it safe and appropriate.

    3. 3Escalation and the emergency boundary

Staff follow a documented escalation process, but emergencies must go directly to the emergency services.

    4. 4What is recorded during care

Managed care is recorded against the care plan and reviewed when needs, risks or circumstances change.
